Applying for WIC

- What to Bring to Your Appointment -

A new client appointment requires approximately 45 minutes.  To schedule an appointment:

    For Mount Vernon and Anacortes, call (360) 416-7595

    For NAS Whidbey Island, call (360) 257-8049

This appointment time is set aside just for you.  Please be sure to call and cancel if you are unable to make the scheduled appointment.  If you are late, the appointment will not be able to be completed.

Initial and recertification appointments are individualized to the family.  The appointment involves measurement of height and weight, and a "finger poke" is done for iron screen.  Medical history and diet are reviewed, and the client is offered individualized referral information, nutrition education, and breastfeeding information.

Please plan to bring:

The individual who is applying (pregnant woman, baby, child)

bullet

Proof of identification for that individual

bullet

Proof of pregnancy if pregnant woman

bullet

Proof of income (see income guidelines)

bullet

Proof of current address

bullet

Diaper, if necessary

bullet

Diet recall for the day prior to the appointment

Please call the office to discuss problems with any of the above items.

Other appointments:  All clients are required by federal guidelines to have an initial appointment and a second appointment for nutrition education during the 6-month certification period.  Other types of appointments include:

Nutrition education classes -- usually about 30 minutes

bullet

Appointment with a Registered Dietitian -- 30-60 minutes

bullet

Infant health assessment at six months of age -- 30 minutes

bullet

One-to-one second contact -- 15 - 30 minutes
